CBHH is a short term acute care psychiatric hospital. Assessments review mental, social, and physical health and are completed by a psychiatrist, clinical and advanced practice care nurses, medical physicians and rehabilitation therapist. The program creates an individualized discharge plan and collaborates with patients, families and significant others along with county mental health case managers and social workers when needed.
